How to Enroll
Step One
Check Eligibility
Our services are available to families residing in McKinley and San Juan counties with children under five years old or expecting.
Step Two
Complete Entry Form
Kindly fill out our recommendation form. Alternatively, you can also submit your own referral form.
Step Three
Submit Form
After you have completed the form, please send it to our fax number at 505-675-2812 or drop it off at one of our offices. Once we have received and reviewed your form, we will contact you by phone.
Step Four
Schedule an Informational Visit
After we connect by phone and you express interest in our services, we’ll schedule an informational visit. During this visit, you will receive all the necessary details about our services.
Step Five
Enroll into the Program
After the informational visit, if you decide to enroll, we will provide you with a registration form. Once you have completed the form, we will enroll you into our program.
